CloseThis Web- Based Entity Privacy Notice or "Notice" discloses eHealthInsurance Services, Inc.'s or "eHealth's" information privacy practices when we assist you in applying for advance payments of premium tax credits and cost-sharing reductions for Qualified Health Plans or "QHPs" and/or in completing enrollments in QHPs through the Federally-facilitated Exchange or "FFE." Throughout this Notice, we will collectively refer to all the activities described in the previous sentence as "QHP enrollment activities."
Throughout this Notice, we refer to information that can identify you as a specific individual, such as your name, email address, or Social Security Number, as "personal information.

Authorization. eHealth
entered into agreements with the Centers for Medicare and Medicaid Services or
"CMS" that authorize us to create, collect, disclose, access, maintain, store
and use your personal information in connection with assisting you in QHP
enrollment activities.

Purpose
for Use and Disclosure. We may
create, collect, disclose, access, maintain, store and use your personal
information for these authorized purposes:
. Assisting with your application for QHP eligibility;
. Assisting with your selection and comparison of QHPs;
. Assisting with your application for advance payments of premium tax credits or cost sharing reductions, and selection of advance premium tax credit amounts;
. Assisting with any appeals you may make of eligibility determination;
. Transmitting your decisions regarding QHP enrollment, advance payments of premium tax credits or cost-sharing reductions to the FFE;
. Assisting with your disenrollment from a QHP;
. Educating you about insurance affordability programs and, if applicable, informing you about your eligibility for programs such as Medicaid and CHIP;
. Assisting with reporting any changes that may affect your eligibility to the FFE;
. Correcting errors in your QHP enrollment application;
. Communicating with you when your QHP is up for renewal, when any individual enrolled in your QHP is no longer eligible because of age or to inform you of QHP coverage options at renewal;
. Providing you with information and materials that we believe can help you learn about the services and options offered through your QHP and other available QHPs, and educate you on how to utilize your QHP;
. Contacting you to assess your satisfaction with or resolve complaints about our service or your QHP;
. Assisting you in communicating with the insurance company that issued your QHP or "QHP Issuer;"
. Carrying out activities related to the efficient operation of your QHP Issuer in the FFE, as required by or permitted by our agreements with your QHP Issuer;
. When required by law; and
. Other activities that are similar to those described above and other activities that may CMS approve in writing.
We will obtain your written authorization before creating, collecting, disclosing, accessing, maintaining, storing or using your personal information in a manner not described in this Notice, and you can revoke this authorization at any time.
3. Your Consent. When you create an account on our website or otherwise provide us with your personal information in order to request our assistance in QHP enrollment activities, you consent to the terms of this Notice. If you disagree with the terms of this Notice, please do not request our assistance in QHP enrollment activities.

Complaints. You have the right to file a complaint with
us and with the Secretary of the U.S. Department of Health Services if you
believe your privacy rights have been violated. You can get a copy of a
complaint form by contacting us, or you can send us a letter telling us about
your complaint. You will not be retaliated against in any way if you file a
complaint.

the right to change this Notice from time. If we make changes this Notice, we

8. Scope of Notice. This Notice describes the information privacy practices that apply when eHealth assists you in QHP enrollment activities. We begin collecting personal information from you under this Notice when you enter your email address, choose your password and click on the "Create an Account" in order to initiate QHP enrollment activities through eHealth. This Notice does not apply to personal information that you may provide to us prior to initiating QHP enrollment activities through eHealth, such as when you provide us with your email address to receive updates on our products and services. Further, this Notice does not cover any personal information that you may provide when applying for other products and services offered by us. Please refer to eHealth's Privacy Policy for a description of the information privacy practices that apply in these other situations.
